I am either the participant or the Parent/Guardian of the above child(ren). This Liability Release is signed in my individual capacity, as well as on behalf of my spouse and my child(ren). This Release forms a binding agreement between all of these persons and Calvary Chapel Dallas.
Liability Release
I hereby release Calvary Chapel Dallas (CCD) in whole or in part and I do hereby assume full responsibility for any and all damages, injuries (including death), or losses that I or my child may sustain or incur, if any, while attending, participating in, or witnessing any activity occurring in or about the premises at 5930 Alma Drive, Plano, Texas 75023 or at any offsite location and agree to release, indemnify, defend and otherwise hold harmless CCD and owners, officers, directors, agents, employees, volunteers, affiliates, and lessees from all costs, losses, damages, liabilities or expenses of any nature, including attorney's fees arising from or in any way connected with any proceeding (legal or equitable) or claim brought or asserted against CCD and its owners, officers, directors, agents, employees, volunteers, affiliates, and lessees arising from or connected with any negligent or wrongful acts or omissions. I acknowledge that my, or my child(ren)s, attendance, participation or witnessing of any activity or event at CCD involves risk and may result in injury, up to and including death.
In consideration of mine or my child’s participation in and the use of the church’s facilities, I hereby release and covenant not to sue CCD, owners, officers, directors, agents, employees, volunteers, affiliates, and lessees from any and all claims resulting from any physical injury that may occur to me or my child while participating in any program or event sponsored by CCD. I am solely responsible for the well-being of myself and my family. This Liability Release is valid unless revoked.
I HAVE READ AND FULLY UNDERSTAND THE ABOVE RELEASE/WAIVER AND FULLY UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING THIS WAIVER VOLUNTARILY. PARENTS OR GUARDIANS MUST SIGN IF APPLICANT IS UNDER 18.
